ORDER FOR COMPLETING POP-OUT WEBPAGES A TO F

• The "New Client Instructions" for processing your Initial Assessment consists of six "pop-out" webpages.  Each of these six pop-out webpages are attached to this webpage and begin with a letter from A to F. 

• Below I have listed each of these lettered webpages, along with the title of their webpages, which if you click on the titles you can go directly to each lettered webpage from this webpage. 
• You can also access webpages A to F by putting your mouse pointer over the words "New Client Instructions"  (this webpage) where is is listed in the Table of Contents and all six webpages will automatically appear.  The Table of Contents is the left column of my website.  

• Each lettered webpage contains specific steps to complete before opening and completing the steps in the next lettered webpage.  It is important for you to complete the instructions on each lettered webpage in the order of their letter and in the numerical order of the steps on each lettered webpage.

• For example, you will begin by opening the pop-out webpage titled "A - Instructions Overviewe" and complete Steps #1 to #3 before opening the pop-out webpage titled "B - Informed Consents" and completing its instructions for Step #4 and #5, and so on.
  

• In other words, you will complete all steps for webpage "A" before completing the steps in webpage "B" and so on until you have completed all steps in webpage "F" - the last of the six pop-out webpages. 

• Once you have completed all steps on each lettered pop-out webpages from A to F, you will:
• Know how to prepare for, collect and send your specimen samples to our Labs or fasting to have blood drawn, whichever applies to you,
• Have printed PDF documents for Health and Symptom History Questionnaires and Forms to fill in the blanks and either email, fax or U.S. mail the completed documents to us.
• Be informed of our policies, as well as have read educational articles that will save you time and money once you begin your therapy and consultations. 

Completing all instructions in order of the Letter for the each pop-out webpage and
 then the  number for each step in the lettered webpage is very important.

A - INSTRUCTIONS OVERVIEW

Webpage "A" provides an overview of the Initial Assessment Processing, Welcome Letter and a Calendar of Events so you will know the time-frame for each step in this process for both our services and completing your instructions.  


B - INFORMED CONSENTS

  Webpage "B" provides the two Informed Consents to print, complete and U.S. mail the orginals to us within 24 hours.  


C - SPECIMEN COLLECTION

  Webpage "C" provides Part 1 of the "Specimen Collection" instructions to prepare for, collect and mail your specimen samples to our Labs and/or having blood drawn.  Reading this information before you receive your Lab Kits and/or Lab Requisitions will save time, and you will be ready when they are received for Part 2 of the Specimen Collection instructions, which is included:

• in the actual Lab Kits that you receive by mail from us, when having hair, saliva, urine or stool testing and/or
• in the Lab Requisition email, when having blood testing.    

D - QUESTIONNAIRES

 Webpage "D" provides PDF documents of each Health History and Symptom Questionnaire and Form to complete and sent to us by email, fax or U.S. mail, within 10 days after mailing your samples to the their Labs and/or having blood drawn.   Properly completing and receiving these questionnaires and forms are required prior to the analysis of Lab Reports and the subsequent releasing of Lab Reports to clients.


E - APPTS & SHIPMENTS

Webpage "E" provides the guidelines, policies and procedures for Telephone and Email Consultations, as well as for Auto-Shipment of your therapeutic whole food supplements and special orders, such as personal skin and hair care, Electro-Magnetic Fields (EMF), and other health maintenance products.


F - CLIENT ORIENTATION

Webpage "F" provides specific educational and informative articles to initiate New Client Orientation.  Reading these documents lays a foundation of knowledge that your consultations will build on for the goal of improving and maintaing your health, as well as help you save time and money at future consultations.   Having these articles read before your Report of Finding Telephone Consultation, which is scheduled to discuss your initial test results, is ideal.  However, if you require more time, having them read before your first monthly Progress Reporting Telephone Consultation is the next best time for these last steps in your "New Client" Instructions.
