ABOUT HAIR SAMPLING web page provides the instructions for preparing the client's hair to collect hair sample when their Lab Kit for the Hair Tissue Mineral Analysis (HTMA) arrives. Note: We may abbreviate this as HTMA for Hair Tissue Mineral Analysis or TMA for Tissue Mineral Analysis, periodically.
FASTING INSTRUCTIONS web page provides the instructions for fasting, starting the day before blood is drawn at a local Laboratory.
BLOOD TESTS LISTS & DR LTR web page provides a list of all blood tests ordered and analyzed by Dr. Smith in the Comprehensive Nutritional Blood Chemistry Analysis (NBCA) and a Physician's Letter from Dr. Smith. When a client will use these two documents is explained on this web page.
CPT CODES & ICD-9 CODES provides the Insurance CPT Codes for our services and ICD-9 Codes are provided by the client's physician, which provides a number for each diagnosis.

preparation and collection of your specimen sample(s),
completing and submitting Health and Symptom History Questionnaires and Informed Consents
and mailing samples to our Labs, when applicable.
