(Open, Type, Print) under title of any form or questionnaire means you can type your information directly into the document before printing   However, you cannot save your information, so please print before closing document.   In due time, all questionnaire and form documents will be revised in this manner.  Print two copies of any document to be U.S. mailed to our office.  Print one copy if faxing or scanning to email.  By printing two copies, you have one for your records for future reference, when needed.
