Update Questionnaire on every Column #1 Date on the Auto-Ship Schedule.
Send completed Dysbiosis Questionnaires by fax, email or U.S. mail to A.C.N.
When Hx Box which represents the Total Score for Historical Points, i.e., "Have you" or "Did You" Questions (#1 to #7, #10 and #15) in Section A - History" is equal to the Grand Score (GT Box), the Dysbiosis Questionnaire is no longer required monthly.
Dr. Smith will monitor your monthly completed questionnaire answers and scores at no additional charge.
A fee-based Dysbiosis Program Progress Reporting Consultation will be scheduled if scores do not decrease from month to month, otherwise no news is good news.
