Completing The Procedures Below Is To Close Clinical Nutrition Therapy In Good Standing
PURPOSE OF CLOSING THERAPY IN GOOD STANDING
Closure of "Therapeutic" Services in "Good Standing" means that the client has completing the three steps below. By doing so, the client is showing respect for the professional therapeutic services and care Dr. Smith has provided and thus, the client is welcomed to request "therapeutic" services again in the
future, wherever the client needs Dr. Smith help, such as:
when sick and does not want to have to use pharmaceutical or nutritional drugs. As you know, nutritional drugs are synthetic or isolated vitamins and minerals, which are the only types sold to the public today. Either type of drug suppresses symptoms making the person using them think they have healed themselves, but they are actually manipulating their biochemistry which will, in time, cause more health challenges.
needs a one-time or short-term therapeutic program for any health challenge that arises.
Because the client knows from their most recent Lab Tests that though some
areas of their body have healed, they still have others that need to be healed,
therefore, if their goal or standard of living is still to obtain optimal over-all body healing, closing therapy now in good standing allows them to return at some future time to resume clinical nutrition therapy.

Ideal Time For Closure The ideal time for closure of services is at the Report of Findings for updated lab testing. If a client is discontinuing their therapy prior to their next Report of Findings, the Therapy Closure Procedures Step #1 is a secondary solution. Why is the Report of Findings the ideal time for closing therapy?
The client will have completed the healing in the areas that their current supplement program has been working on since their last Lab Tests. Otherwise, areas that were being healed by these supplements will regress once supplements have been discontinued.
Both Dr. Smith and the client have scientific lab testing of the client's nutritional biochemistry progress at the time of closure. In other words, at the time of closing therapy, they know exactly which areas of the body have healed and which have not yet healed.
This also allows the client to know which areas of the body they need to keep an eye on so they do not wait too long to resume therapy and are surprised by an unexpected disease in the areas not yet healed.

HAC Report - Please complete and return the Health Appraisal
Comprehensive (HAC) Questionnaire within the next 10 days. Why is this important for proper closure?
The HAC Report, though not a scientific "test" like a Laboratory Report, it does provide a scientific "evaluation" of your symptoms progress, since the last time you received a HAC Report, at least.
Both you and Dr. Smith will know which symptoms have abated (perfect score of zero) and to what degree other symptoms had improved at the time of closure.
If/when therapeutic services are provided
again in the future, your updated HAC Report, at that time, can be accurately compared to your symptoms on your closure HAC Report. Thus, you and Dr. Smith will then be able to determine:
how well you have integrated and consistently maintained the dietary and lifestyle education you have received, from a symptoms perspective,
if any new symptoms that may have arisen from exposure to environmental pollutants outside your control.
the degree of severity symptoms have progressed in areas of the body that have not yet received clinical nutrition therapy.
Should you chose, instead, to resume therapy with another provider, this Closure HAC Report will you determine the effectiveness of the new provider's therapy, from a symptoms perspective, at least. Note: Because the HAC Report would not be provided by another provider, Dr. Smith would be glad to periodically provide this service for an accurate comparison, upon request.
Therapy Closure Telephone Consultation - Upon receipt of client 's completed HAC Questionnaire, the closure telephone consultation with Dr. Smith is scheduled and client must complete both questionnaire and consultation to conclude "therapy" closure in good standing. At this consultation, Dr. Smith will provide the following:
an overview of what was accomplished through our clinical nutrition therapeutic services.
comparison of biochemical progress from the initial to the closing biochemical testing, if lab tests were updated. In other words, which areas are healed, which areas still need to be healed (in order of priority).
comparison of symptoms progress from the previous HAC Report to the closing HAC Report, i.e., which symptoms have abated (gone) and which have improved since last HAC Report. This also indicates which areas have new or increased symptoms scoring because these areas have not yet been provided therapy due to either
providing therapy on areas of higher priority or
Supplement Financial Plan (Turtle, Squirrel or Rabbit) did not allow finances to work on these areas by time of closure.
instructions in weaning off on-hand therapeutic
supplements in order to not upset or adversely affect biochemistry. (Refer to Step #3) Just like weaning from drugs, if not done properly, can have serious consequences. Inventory Email - Client will provide an inventory of their on-hand
supplements prior to the Therapy Closure appointment, so Dr. Smith may design a weaning
supplement program of care as explained in Step #2 - 4.

PROPER CLOSURE IS ALSO A HEALING THERAPY FOR CLIENT AND DOCTOR
Most of what has been communicated above shares the benefits
to the client for being an honorable person and showing respect for the therapeutic services Dr. Smith has provided, to date, by properly closing. At this time, we would like to
communicate a benefit from Dr. Smith's perspective. Throughout the
period of providing therapy to each client, Dr. Smith invests much
time, energy, thought, prayer, care and money in advancing training and knowledge, and when client need arises, even goes beyond what is expected in a doctor-client relationship, that extra mile, so to speak.
Therefore, for a client to
discontinue services without at least providing a HAC questionnaire, so
Dr. Smith can assess what has been accomplished by all her efforts, at least, in respect to symptom improvement, is very
unsettling. Also, lack of proper closure is mentally and emotionally unhealthy for both Doctor and Client, though clients may not be aware that this is unhealthy for them, also. In other words, Mental and emotionally unresolved events, unanswered questions, abrupt ending of relationships, not being given the opportunity to express etc., cause physical illnesses too. Hopefully, as you come to know Dr. Smith, you know her intentions are to do what is right and healthy in all situations to the best of her ability.
Therefore, Mental and Emotional Health requires proper closure, also, whether that is in a
relationship between two friends, discontinuing employment in favor of a
new job or in this case, knowing what has been accomplished by one's efforts. So
proper closure is also a healing therapy for both client and doctor.
